There is mention near the end of the segment that if you supervise PA's or residents and deleting information to make the chart look good that this shouldn't be done. You did make mention of doing an addendum to the chart if something wasn't entered correctly by a scribe. However, is it legal to actually delete data entered by another provider?

Joe,
I don't know of any legal issues that come up when you are cosigning a chart and putting an attestation that you saw the patient with an advanced practice provider. Obviously if you are just cosigning charts and not documenting/billing as if you personally saw the patient I wouldn't make any changes but in most cases where you are supervising and cosigning there shouldn't be any issues with addition additional information to the chart to accurately reflect your recollection of the case.
