Thank you guys so much for this today! I had an elderly pt with a history of BPH/prostate CA come in today with UTI symptoms and I knew EVERYTHING to do!!! This was perfect! Better yet, it is now solidified in my brain :)

On a more serious note learning from others mistakes is ok if nothing disastrous happens but when there is a death or serious disability caused it much much harder to share .
There ought be a anonymous pool of Great Saves and Catastrophic Errors .
Patient or Family compensation ought be initiated but Confrontational Law Suits abandoned except in cases of Malicious injury .
