The new AAP policy statement recommends isotonic fluids for hospitalized children 28 days to 18 years of age.

These recommendations do not apply to neurosurgical disorders, congenital or acquired cardiac disease, hepatic disease, cancer, renal dysfunction, diabetes insipidus, voluminous watery diarrhea, or severe burns.

No recommendation in regards to the specific composition of the fluid (NS, LR, Plasmalyte) is made though studies in adults suggest NS leads to higher rates of renal replacement therapy and overall mortality.
