I have a content question....
In the May edition of Peds RAP (Chapter 4 - Asthma), the guest said that when considering whether family members of asthmatics were smokers, that outcomes for those who smoke in the home and those who smoke outside were the same (and worse than non-smoking families). I have not been able to locate a reference for that - can you help?

I believe the reference is:
Gill R, Krishnan S, Dozor AJ.J. Low-level environmental tobacco smoke exposure and inflammatory biomarkers in children with asthma. Asthma. 2014 May;51(4):355-9. doi: 10.3109/02770903.2013.823446. Epub 2014 Mar 7.

It showed elevated urinary markers of nicotine byproducts (urinary cotinine) in a high # (70%) of kids whose parents smoked "outside the home"...hope that helps!

There is also this study which shows worse PFT's in most kids after similar exposures...:
