In your "red hot knee" mailbag question I kept waiting for a mention of Lyme disease. Living in the northeast (CT), my experience with unilateral "red hot knee" with minimal trauma has been "Lyme disease wins 5-0 over septic knee arthritis!" Obviously, one wouldn't want to assume it's Lyme and not assess for septic knee, but Lyme would always be on my differential; I've seen it several times as the first presentation of Lyme (no malaise, no EM rash).
