Well you ought trial PPIs to protect eg patient history of peptic ulcer or erosive esophagitis even double dose PPIs like pentrazapole (Somac) less interaction with warfarin .
As for the 250 falls have not read result of fall eg bruise or subdural hematoma or # femur with 2L blood loss .
Clearly the best is intense falls prevention protocols where fragility index reaches a level say X n
