The summary below is from an episode of ERcast: Clinical Perspectives

COVID-19 in pregnancy increases maternal and neonatal risk, while mRNA vaccine safety data in pregnant patients remain reassuring. The key questions are severe maternal outcomes, preterm birth, stillbirth, and whether vaccination signals miscarriage, congenital anomalies, or other major obstetric harm.

COVID Risks and Vaccine Safety

  • Maternal disease severity: Pregnancy is a high-risk state for severe COVID-19, with higher rates of ICU-level illness, ventilation, and maternal morbidity than in nonpregnant peers.
  • Neonatal outcome burden: Maternal infection tracks with worse neonatal outcomes, especially preterm birth and downstream NICU utilization, a distinction we put into practical counseling language in the episode.
  • mRNA vaccine reassurance: Large observational pregnancy datasets have not shown a major safety signal for mRNA vaccines, including miscarriage and congenital anomaly concerns.
  • Risk-benefit counseling frame: The clean bedside message is that infection risk is measurable and consequential, while vaccine harms have not emerged as a comparable signal in the available data.
  • Patient-centered counseling nuances: Counseling still needs nuance around timing, uncertainty, and individual comorbidity. We get into the phrasing that helps with hesitant patients in the chapter.
